The HBPC RN Care Manager provides care planning and assessment, care coordination, chronic disease management and direct patient care in the homes of Veterans including private residences, residential care facilities, assisted living facilities in collaboration with the HBPC multidisciplinary team. The RN will provide care coordination and direct patient care for veterans residing in multiple geographic locations within Navajo County. The HBPC RN Care Manager role includes initial and ongoing assessment/reassessment, monitoring and care coordination during the veteran's entire enrollment period to meet the patient's healthcare needs utilizing the principles of case management. The HBPC RN Care Manager ensures smooth transition of care to and from home through diligent and collaborative efforts with multidisciplinary teams within and outside the facility. This would also include co-management of care with outside community agencies. The program provides for longitudinal care whereby the veteran may stay in the program for the duration of his lifetime. The HBPC RN Care Manager provides direct patient care to the veteran's residences that require routine periods of traveling built into their tour of duty. This requires the RN to drive distances based on established radius from an assigned VA location. Work Schedule:
40 hours, Monday - Friday, from 7:
00am - 3:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Experience:
BSN Prior community health or home care experience. Documented evidence of satisfactory performance and dependability. Minimum of two years recent clinical nursing experience with documented evidence of good clinical, assessment and problem solving skills. Ability to easily establish and maintain good interpersonal relationships and communication with physicians, staff, Veterans/caregivers, and community healthcare and resource providers. Demonstrated ability to perform duties with initiative and independence. Ability to communicate effectively both verbally and in writing. Skill in the use of CPRS for documentation and accessing patient data is preferred. Computer literacy is essential as staff utilize an electronic medical record system. Current Arizona driver's license is required. Government vehicles are used in making home visits. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. If a current VA employee is selected, he/she will reassign at their current grade and step, and placed on the assistant nurse manager pay schedule. Reference:

Near vision correctable at 13 to 16; far vision correctable in one eye to 20/25 or better. Ability to distinguish shades of color is not required but would be beneficial. Incumbent will sit, type, walk and talk with staff, manage databases. S/He should be able to carry 20 lbs for long distances in extreme heat on an intermittent basis. The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing (steps, ladder, reaching (overhead, extensive/repetitive), repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking. The environment's pace can vary from steady to a sometimes fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials.

The check will bounce and the victim is left responsible.
